do I have to take off the inside of the tailgate or can I replace from the outside.it looks like there are a couple of screws holding it
Which version qx do you have? The first gen, yes you have to take off the back door finisher. The service manual can be found online for free and will show you all the steps.

Not hard at all, just did this earlier this year. Speaking for a 2011 qx56 you have to take off the finisher on the inside of the lift gate. There should be about three are four 10mm bolts to undo. Once you undo them and unhook the wiring harness, the whole trim assembly should come right out. For the camera itself it is held in by two Phillips head screws.
